Dementia/Alzheimer’s Care
Although we do not have a secured unit at Brookfield Post Acute, we do offer Dementia/Alzheimer’s care services including but not limited to the following:
- Supervision and Support: Continuous monitoring to ensure the safety and well-being of residents.
- Personalized Care Plans: Tailored care plans to meet the unique needs of each resident, including assistance with daily activities such as bathing, dressing, and eating.
- Cognitive Stimulation: Activities designed to engage residents and help maintain cognitive function, such as memory games, music therapy, and social interactions.
- Behavioral Management: Strategies to manage and reduce challenging behaviors associated with dementia, such as agitation and wandering.
- Medical Management: Regular health assessments and medication management to address the medical needs of residents.
Complex Wound Care
Our wound care program utilizes professional, clinical, and technical resources designed for the treatment of a variety of wound conditions including ostomies (colostomy/urostomy), wound separation, diabetic arterial/venous ulcers, pressure ulcers, and pre- and post-flap surgery.
Respite Care
Through our respite program, our facility provides security and supervision in a caring and compassionate environment, giving the caregiver peace of mind. Respite care is designed to meet the physical and medical needs of a resident during a short-term stay.
Diabetes Care
Our experienced nurses provide help with all aspects of managing diabetes including blood glucose monitoring, insulin administration and tracking, exercise planning, menu planning, and education.
